Mesothelioma is a terrible disease that has a huge latency period. Stage I: At this stage, the presence of mesothelioma is in the right side or the left side of the pleura (lungs). It may also be in the pericardium or diaphragm. Diaphragm is the muscle that separates the chest and the abdomen. In this stage, mesothelioma is only on one side (left or right) of the pleura.

Stage II: In this degree of mesothelioma, the disease spreads to the other side of pleura or to the esophagus or heart or chest wall or even the lymph nodes.

Stage III: In this phase, the disease is in a serious assaulting phase. It spreads from the diaphragm into the peritoneum. Peritoneum refers to the abdominal cavity lining. While in the previous stage, the lymph modes of the chest may be involved. This stage may involve the lymph nodes beyond the chest too.

Stage IV: This is the final terrible stage of mesothelioma wherein the disease spreads to the other organs through the bloodstream.

TNM staging System

The American Joint Committee on Cancer (AJCC) has developed another new staging system. This TNM staging system is very similar to that used for other types of cancers. In this system T stands for tumor

N stands for the assault on lymph nodes and M stands for metasis.

The staging system is based on the following: § the size of the tumor and the extent to which it has spread to nearby organs § the extent of spread to lymph nodes § the extent of spread to distant organs.

The various degrees of mesothelioma as per the TNM staging system are as follows:

Stage I: The mesothelioma may be in the right or the left pleura. It may also have assaulted the pericardium or the diaphragm on the same side. However, it would not have spread to the lymph nodes.

Stage II: The disease may have spread to the lungs, diaphragm, lymph nodes or pericardium on the same side.

Stage III: In this phase, the mesothelioma may have spread into the muscles of the chest wall, ribs, esophagus, heart and other organs on the same side. It may also have spread further into the lymph nodes on the same side.

Stage IV: In this stage, mesothelioma affects the lymph modes in the chest on the same side or may start spreading to the lungs or pleura on the other side. Also, if mesothelioma is found in the distant organs or if it spreads to organs other than the abdomen and chest, (through the blood stream), it is the fourth stage.

Though the TNM system is more clear and comprehensive, still the Butchart system is popularly used to describe the stages of mesothelioma. Whatever the staging system used, it is important to understand that mesothelioma stage depiction is important for deciding the type of treatment to be used on a patient.

How are the stages of mesothelioma determined?

CAT scans, MRI testing and other tests like x-rays help to determine the stage of mesothelioma in the patient. After this, the doctor informs the patient and his or her family about the level and type of treatment that is required. Mesothelioma has no known cure. Erlotinib (Tarceva), which blocks chemicals that signal the cancer cells to grow and divide. Erlotinib is approved for people with advanced and recurrent non-small cell lung cancer that haven't been helped by chemotherapy. Erlotinib side effects include a skin rash and diarrhea.

Doctors are getting better at diagnosing and treating lung cancer, which means people with lung cancer have a better chance of recovering and living longer. The surgical incisions made during VAT are more minor than those needed for thoracotomy or sternotomy.

However, physicians warn that VAT does not actually allow complete lung examination to identify and remove metastases that are not detected by preoperative chest X-ray. VAT is appropriate for Stage 1 and Stage 2 cancers that require lobectomy (surgical removal of a lung lobule) with lymphadenectomy (removal of one or more lymph nodes) and for peripheral (outer edge) lung tumors that can be removed by wedge resection.

Chemotherapy is the best option together with radiotherapy if the tumor is more aggressive and widespread.

Photodynamic therapy is most suitable for patients having inoperable lung cancer. This begins with the shot of a light-activated drug (e.g., photofrin/polyhaematoporphyrin, lumin). Then during examination of the airways using a flexible scope the lung tumor is illuminated by a laser that transmits light of a specific wavelength. The laser light is used to wipe out the sensitized tumor tissue.

Skin photosensitivity or light sensitivity is the side effect of PDT. The healing potential of PDT is the most exciting aspect of this therapy in lung cancer patients whose tumors are unseen on chest X-rays. The tissue-sparing effects of PDT may be particularly important for individuals with limited lung function.

Electrosurgery is performed using a needle, bulb, or disk electrode. Nd-YAG laser therapy (neodymium-yttrium/argon laser that concentrates high-energy electromagnetic radiation to destroy tissue), cryotherapy (destruction of tissue using extreme cold), and brachytherapy (treatment with ionizing radiation) are extra tumor size-reducing techniques that may be performed during bronchoscopy.

Lung Cancer Treatment -- Photodynamic Therapy Offers a Safe Alternative

Photodynamic therapy (PDT) is one of the newer lung cancer treatment options available today. Lung cancers are sometimes inoperable, particularly when they are situated in the bronchi or trachea. Similarly, cancers that have spread from other parts of the body to the bronchi cannot be removed surgically. In such cases, photodynamic therapy offers a safe and effective treatment option.

In a best-case scenario, PDT is used as a curative therapy that can eliminate the cancer completely. In many patients, PDT is employed as a palliative lung cancer treatment meant to provide relief from symptoms even though it does not cure the cancer.

Photodynamic therapy can effectively relieve symptoms like breathing difficulties from obstructions in the bronchi or trachea. In some patients, tumors block the airways and can cause coughing, breathing difficulties, pneumonia and bleeding. PDT may be used in such cases depending on the location, size and stage of the tumor.

PDT destroys cancer cells using a laser. To ensure that only cancerous cells are destroyed, a special drug is used to make the cancer cells more sensitive to this treatment while leaving normal cells intact.

There are three steps involved in the process of lung cancer treatment using PDT. In stage one, a drug called photofrin is injected intravenously. This drug has the property of making body cells very sensitive to light. Normal, healthy cells eliminate photofrin, but cancerous cells are unable to do so and the drug is retained in such cells.

In the second stage of PDT, about forty to fifty hours after the injection is given, a flexible tube is inserted into the bronchia. This tube contains a red laser of low intensity. The cancer cells exposed to this laser are destroyed.

In the third and final stage, about two days after the laser light exposure, a bronchoscopy is done to get rid of the dead cancer cells and mucus from the bronchi or trachea.

A patient can benefit from photodynamic therapy only if his or her cancer is situated in a location that is accessible to a bronchoscope. Also, the tumor must be large enough so that the bronchoscope can easily detect it. This means that some cancers like those located outside the bronchial passages cannot be treated with PDT.

Lung cancer treatment with PDT offers almost no dangerous side effects, unlike other treatment methods. Photosensitivity is about the only side effect that lasts for a while. Since photofrin makes cells sensitive to light, the patient may experience heightened light sensitivity for a period of four to eight weeks after the treatment.

During this time, even normal exposure to sunlight can result in severe sunburn. For this reason, patients who have undergone PDT are advised not to venture out into the sun for about eight weeks after their lung cancer treatment. In contrast to photodynamic therapy, major surgery done to eliminate lung cancer usually poses much greater risks.

Many tumors that are inoperable because of their location are prime candidates for treatment with PDT. When used as a palliative, this type of lung cancer treatment offers almost immediate relief.

Given all these advantages, increasing numbers of patients have been using photodynamic therapy as a lung cancer treatment of choice.
